Please complete the forms that have are provided below in pen. If for any reason you are unable to complete the forms or you have any questions regarding them, please call our office at (403) 278-1415.
Due to the privacy legislation it is your responsibility to obtain a copy of your most recent pan x-ray and any x-rays taken within the last 12 months. If we do not receive these x-rays at the time of your appointment we will take the necessary x-rays at that time.
